My right shoulder got dislocated while playing badminton.----Remedial Measure.?

It got dislocated 20 times from May 1984 to till date.All ligaments got treared off.While moving the shoulder i experince more friction and continuous pain neck to shoulder.I I have diabetics from last 11 years.Please advice me what i have to do now.
